  * first occurrence
  * last occurrence
  * all occurrences

Add user option to select displaying contents (from the following) when listing concepts:
  * from "tval_char", or "nval_num" (if former is blank), or "concept_cd" (if prev 2 are blank)
  * from "concept_cd"
