 Description   
The c_dimcode field for Age at visit is incorrect for ages 24, 80, 81 and 89.
An AgeAtVisitUpgrade.sql script was created that should run on sqlserver only.

This script should run on SQLSERVER only. Postgres computes age differently and oracle does not have the bug.

on new installs no new data install path is needed

However release notes need to mention a new data upgrade path for SQLSERVER only.
